A trained qualified competent healthcare provider. Who provides and coordinates care, counsel and prevent adverse events, and optimisation of care service productivity in clients outcomes. She measures, monitors and report on the appropriateness and effectiveness of care, informing improvements in the care we provide quality. She supports, in implementation and evaluation of clients requests to improve their safety and participation providing an individual centred-care. Responsibilities include client-centred care through our systems and processes that support shared continuity of care, open disclosure, and sensitivity to the cultural needs and health literacy of clients.
